PARTO NORMAL E ESTADO NUTRICIONAL ADEQUADO DA MÃE COMO FATORES PROTETORES CONTRA O ALTO ÍNDICE DE MASSA CORPORAL EM ESCOLARES

Resumo
Introdução: A obesidade é uma doença multifatorial, caracterizada em grande parte pelo estilo de vida materno. Nesse contexto, o tipo de parto e o estado nutricional materno vêm sendo investigados como determinantes diretos na adiposidade dos filhos. Objetivos: Verificar a influência do estado nutricional da mãe na relação entre o tipo de parto com o índice de massa corporal (IMC) de escolares, considerando a influência de potenciais confundidores. Material e Métodos: Estudo de coorte transversal, com 1379 crianças e adolescentes, com idades entre 6 e 17 anos, de escolas públicas e privadas de Santa Cruz do Sul-RS. Peso e altura dos escolares foram mensurados em balança antropométrica com estadiômetro acoplado. A partir disso, o IMC foi calculado através da razão entre peso (kg) pela altura (m). Informações acerca do tipo de parto, peso e altura para cálculo do IMC materno, foram obtidas através de questionário autorreferido. O IMC materno foi classificado em peso normal e excesso de peso. Para a análise dos dados, foi utilizada regressão linear generalizada, considerando significativo um p<0,05. As análises foram ajustadas para: sexo, estágio de maturação, idade da mãe, amamentação exclusiva, peso ao nascer, idade e nível socioeconômico. Resultados: Os dados indicaram que 49,5% das mães tiveram parto normal, e 43% dos escolares apresentaram excesso de peso. Além disso, foi encontrada associação inversa entre parto normal com o IMC de escolares (ß=-0,56; IC95%=(-1,00 -0,12); p=0,01). No que se refere a influência do IMC materno nessa relação, houve associação inversa entre parto normal com IMC apenas nas mães que apresentaram peso normal (ß=-0,64; IC95%=(-1,18 -0,10); p=0,01), considerando a influência de diferentes fatores de confusão. Já nas mães com excesso de peso, não houve associação significativa (ß=-0,45; IC95%=(-1,08 0.17); p=0,16). Conclusão: A obesidade da mãe influencia na relação entre o tipo de parto e IMC dos seus filhos. Portanto, para que haja uma influência positiva no IMC dos seus filhos, a mãe deve preconizar o parto normal, bem como um estado nutricional adequado.
