Fatores de risco, Infecção hospitalar, Resistência bacteriana
Resumo
Introdução: As infecções relacionadas à assistência à saúde (IRAS) provocadas por microrganismos resistentes ainda é um grande problema nas unidades hospitalares, aumentando tanto o tempo de permanência do paciente, como também, o risco de vida. Objetivo: Identificar as bactérias multirresistentes encontradas em pacientes internados em um hospital de ensino no interior do Ceará. Material e métodos: Os dados coletados foram referentes ao período de janeiro a dezembro de 2020. A coleta de dados foi realizada no sistema da Comissão de Controle de Infecção relacionadas à Assistência à Saúde. Ressalta-se que a identificação e teste de sensibilidade antimicrobiana dos materiais biológicos dos pacientes foram analisados no Laboratório de Microbiologia da instituição utilizando o VITEK® e BacT/ALERT® conforme padronização do Clinical and Laboratory Standards Institute. Não aceso aos dados de pacientes. Resultados: Foram identificadas 95 infecções laboratorialmente confirmadas por bactérias multirresistentes, todas ocasionadas por bactérias bacilos gram-negativos. Essas IRAS representaram 9% em relação ao total de notificações e foram constituídas por 9 bactérias diferentes. Os bacilos gram-negativos mais prevalentes foram: Pseudomonas aeruginosa (28,42%) tendo sua maior incidência no mês de março, Klebsiella pneumoniae carbapenemase representou 27,36% e com maior incidência no mês de agosto, Acinotobacter baumannii representando 27,36% tendo maior incidência em janeiro e Proteus mirabilis (8,42%) com maior incidência no mês de março. O microrganismo de maior prevalência em 2020 foi a Pseudomonas aeruginosa (28,42%). As culturas foram de diversos tipos topográficos sendo identificados 42 (42,90%) infecções de corrente sanguínea, 26 (27,46%) infecções do trato urinário, 1 (1,08%) infecção sítio cirúrgico e outros constituindo um total de 28 (28,57%) culturas positivas. Ressalta-se que a classificação de sensibilidade seguiu a orientação da Anvisa. Conclusão: Os dados apresentados são um importante passo para identificar o perfil microbiológico do Hospital e dessa forma planejar ações voltadas à prevenção e ao controle desses eventos adversos.
