Corticosteroides, Gravidez, Infecções por Coronavirus.
Resumo
Introdução: No início de 2020, foi confirmado pela Organização Mundial da Saúde (OMS) a pandemia da doença COVID-19 causada pelo vírus SARS-Cov-2. Por conseguinte, o campo da obstetrícia vêm enfrentando novos desafios, e adaptações são necessárias. O uso de corticoesteroides é comum quando há possiblidade de parto prematuro, garantindo a maturação dos pulmões do feto até o nascimento. No entanto, estudos relacionam resultados negativos entre pacientes com COVID-19 e o uso de corticoesteroides. Objetivo: Levantamento bibliográfico em busca da associação entre a COVID-19 e as possíveis complicações do uso de corticoesteroides em gestantes. Material e métodos: Trata-se de uma revisão integrativa da literatura, realizada a partir de artigos publicados nas bases de dados PUBMED e Google Acadêmico com um recorte temporal de 2019 a 2020. Resultados: Durante a pandemia, é necessário avaliar os riscos e benefícios para mãe e bebê, e quando o uso desses medicamentos é realmente necessário. É recomendado que gestantes com COVID-19 confirmada ou suspeita usem corticoesteroides somente após as 32 semanas de gestação. A tocólise não é recomendada, exceto em casos como a nifedipina, se a mãe não for tiver hipotensão ou taquicardia. Atualmente, a indometacina é um tocolítico anti-inflamatório eficaz usado em substituição dos esteroides. O uso de anti-inflamatórios não esteroides não deve ser alterado no cenário do SARS-Cov-2. Conclusão: É indispensável uma avaliação individual cuidadosa do risco materno versus o benefício neonatal. A equipe multidisciplinar é essencial na decisão sobre o uso de corticoesteróides e tocólise. O uso dos medicamentos nifedipina e indometacina são possibilidades para a tocólise, mas devem ser bem analisados pelos profissionais de saúde.
