AVALIAÇÃO OFTALMOLÓGICA, DIAGNÓSTICO DE DOENÇAS SISTÊMICAS, EXAME OFTALMOLÓGICO, MEDICINA CLÍNICA
Resumo
Introdução: O exame oftalmológico é realizado no intuito de verificar a saúde do sistema visual, composto pelo bulbo ocular, nervo óptico, oculomotor, troclear e abducente, músculos intrínsecos, extrínsecos e a cavidade orbitária. Durante o exame, é possível identificar outras doenças que afetam os sistemas do organismo humano, sobretudo nervoso, endócrino, cardiovascular, além de patologias infecciosas. Objetivo: Compreender a importância do exame oftalmológico para o diagnóstico de doenças sistêmicas. Material e Métodos: Trata-se de estudo descritivo, por meio da revisão bibliográfica, realizada por meio de livros e artigos científicos publicados nos últimos cinco anos disponíveis na plataforma Scientific Electronic Library Online (SciELO) acessíveis na íntegra nas versões em português e inglês. Utilizou-se como palavras-chave “exame do olho”. Foram encontrados 105 resultados, dos quais foram selecionadas 10 obras para compor a revisão bibliográfica. Resultados: Os estudos revelam que a avaliação oftalmológica se inicia após a realização da anamnese, com a inspeção da estrutura ocular e anexos, sendo possível a percepção de sinais como exoftalmia, que é indicativo de doenças tireoidianas. O exame segue com o teste de acuidade visual grosseira, que confere a acurácia e a nitidez da visão do paciente e é feito com a utilização da Tabela de Snellen ou o Cartão de Rozembaum. Esse teste auxilia na detecção de patologias relacionadas à perda da acuidade visual, como sífilis, tuberculose e esclerose múltipla. Por conseguinte, testa-se o campo visual do indivíduo, no qual pode-se encontrar escotomas característicos de HIV e de esclerose múltipla. Por fim, realiza-se a fundoscopia com o auxílio do oftalmoscópio, para checar as estruturas oculares internas, sobretudo a retina e o disco óptico. Nesse momento, retinopatias e lesões podem ser identificadas, o que pode apontar para o diagnóstico de diabetes mellitus, hipertensão arterial sistêmica, leucemia, sífilis e hipertensão intracraniana (por presença de papiledema). Ainda durante o exame, há também a verificação de reflexo pupilar para a identificação de problemas nos músculos oculares e moléstias neurológicas, como aneurismas. Conclusão: Os sinais encontrados na avaliação oftalmológica, associados a outros sintomas, corroboram para o diagnóstico de doenças corpóreas, se consolidando como um exame de grande importância para obter achados clínicos.
