PREVALÊNCIA DA INFECÇÃO POR L. L. chagasi EM PACIENTES POLITRANSFUNDIDOS E CONTROLES: POSSÍVEL CORRELAÇÃO COM TRANSFUSÕES DE HEMOCOMPONENTES CELULARES

Resumo
Introdução: A leishmaniose visceral é causada pela infecção do indivíduo susceptível por alguma leishmania viscerotrópica. Mais de 80% dos indivíduos infectados são assintomáticos, representando uma ameaça à segurança transfusional, uma vez que podem ser reservatórios do parasito. Logo, é necessário investigar formas eficazes e viáveis de detecção do parasito nesses indivíduos. Objetivo: Avaliar a prevalência de infecção por L. L. chagasi em pacientes politransfundidos de região endêmica para leishmaniose visceral e sua possível correlação com a transfusão de hemocomponentes. Material e Métodos: O grupo controle foi composto por 204 indivíduos e o grupo de estudo por 271 indivíduos politransfundidos, todos provenientes de regiões endêmicas para leishmaniose visceral (Montes Claros -MG, Teresina – PI, Fortaleza, Crato e Sobral – CE). O projeto foi aprovado pelo Comitê de Ética da Universidade Federal do Triângulo Mineiro e Fundação Hemominas (2104 e 332). O DNA genômico foi extraído pelo Kit QIAamp® Blood Mini Kit (QIAGEN™). A análise molecular para identificação de Leishmania spp foi realizada por PCR em Tempo Real. A análise estatística foi realizada utilizando-se o BioEstat 5.0 e valores de p <0,05 foram considerados estatisticamente significativos. Resultados: Foram analisadas 475 amostras, sendo 72 de Fortaleza (57 Pacientes e 15 Controles); 86 de Sobral (45 Pacientes e 41 Controles); 49 de Crato (24 Pacientes e 25 Controles); 125 de Montes Claros (68 Pacientes e 57 Controles) e 143 de Teresina (77 Pacientes e 66 Controles). Foram encontradas 12 amostras positivas, sendo 9 Pacientes (4 Fortaleza, 2 Sobral, 3 Montes Claros) e 3 Controles (2 Fortaleza e 1 Teresina). Apesar da prevalência 2,4 vezes maior nos politransfundidos, não foi observada diferença estatisticamente significativa entre os grupos (p=0.33; OR=2,3; IC=0.61-8.61). Conclusão: Houve maior prevalência de indivíduos positivos para L.L.chagasi no grupo politransfundidos em relação ao grupo controle, sugerindo uma possível correlação com a transfusão de hemocomponentes.
