INFLUÊNCIA DO HTLV-1 NO APARECIMENTO DE INFECÇÕES GRAVES POR Strongyloides stercoralis

Resumo

Introdução: Strongyloides stercoralis é o agente etiológico da estrongiloidíase, parasitose que infecta cerca de 100 milhões de pessoas em todo o mundo. Em pacientes imunocompetentes, a patogenia varia de assintomática a desconfortos leves, entretanto, configura-se como um sério problema de saúde pública a partir do agravamento da estrongiloidíase em indivíduos sob condições imunossupressoras, como em uma infecção pelo Vírus Linfotrópico T Tipo 1 Humano, HTLV-1. Objetivo: Compreender o impacto do HTLV-1 durante um quadro de coinfecção por Strongyloides stercoralis. Material e métodos: Foram selecionados artigos científicos correspondentes ao tema das bases de dados científicos PubMed e SciELO. As seguintes palavras-chave foram utilizadas para a pesquisa, nos idiomas Português e Inglês: "Strongyloides stercoralis", "HTLV-1", e "S. stercoralis HTLV-1 coinfecção". Resultados: O vírus HTLV-1 possui tropismo para os linfócitos T CD4+ Th2 (T helper 2), que secreta interleucinas, dentre elas, a IL-4 e IL-5, responsáveis pela replicação de eosinófilos, sendo esses ligados a processos alérgicos e infecções parasitárias. Devido à comprometibilidade dessas células, a possibilidade de rápida reprodução de larvas de S. stercoralis é consideravelmente maior. Além disso, aproximadamente 67% dos casos analisados reportaram reinfecção por Strongyloides stercoralis após o tratamento. Em pacientes hiperinfectados, os principais sintomas incluem: febre, dor de cabeça, desordens gastrointestinais e há quadros de obstrução intestinal pelas larvas. A disseminação de S. stercoralis para outros sítios também é citada na literatura. Embora seja menos frequente, é potencialmente perigosa porque afeta órgãos vitais como, pulmões, cérebro e rins, sendo sua taxa de mortalidade, se não tratada, de 100%. Conclusão: Pacientes HTLV-1 positivos são grupo de risco para parasitoses oportunistas tais como a estrongiloidíase, principalmente em países tropicais, como o Brasil. Portanto, a devida atenção e acompanhamento em casos de coinfecção é fundamental para se obter um diagnóstico preciso e um tratamento eficaz.
