DIFICULDADES NA PRODUÇÃO DE VACINA PROFILÁTICA ANTI-DENGUE: ASPECTOS PATOLÓGICOS E IMUNOLÓGICOS

Resumo
Introdução: A Dengue é uma doença negligenciada de importância mundial que ocorre em Regiões Tropicais e Subtropicais, principalmente em áreas urbanas e semiurbanas, em que, atualmente, quase metade da população mundial encontra-se sob risco de contrair a doença. Causada pelo Vírus Dengue, Arbovírus pertencente à família Flaviviridae, possui 4 sorotipos diferentes. É transmitida através de mosquitos vetores do gênero Aedes, ocasionando manifestações brandas e severas, que podem ou não evoluir à óbito. Entretanto, não possui tratamento específico, sendo utilizada apenas terapia de suporte, e a profilaxia baseia-se sobretudo em controle vetorial; devido a isso, surge a necessidade de desenvolvimento de uma vacina contra a doença. Objetivos: Demonstrar os fatores que dificultam a produção de vacinas profiláticas contra dengue, de forma segura e eficaz. Material e métodos: Revisão bibliográfica utilizando as bases de dados Google Acadêmico, Scielo e PubMed. Foram obtidos 113 artigos científicos em Português, Inglês e Espanhol, publicados entre os anos de 2000 e 2019. Resultados: Desde a década de 1940 há pesquisas para a formulação de vacinas contra dengue e, atualmente, diversas vacinas estão em diferentes níveis de desenvolvimento. Dentre as dificuldades para a produção desta estão a falta de modelos animais, necessidade em abranger todos os sorotipos do vírus, risco em estimular infecções graves e desenvolvimento de proteção duradoura. Conclusão: As vacinas em desenvolvimento avançado ainda apresentam falhas e decorrentes riscos aos indivíduos vacinados, sendo estes derivados da interação entre sorotipos, exposição pós vacinal ao vírus, decaimento da proteção vacinal, quantidade de doses administradas e risco em desenvolver gravidade. Portanto será indispensável uma melhor compreensão acerca da imunidade natural e aquela estimulada pela vacinação, bem como a interação destas.
