Ceratoconjuntivite seca, Síndrome de Sjögren, Síndromes do olho seco, Xeroftalmia, Xerostomia
Resumo
Introdução: A Síndrome de Sjogren é uma doença autoimune crônica, lentamente progressiva, decorrente da infiltração linfocitária das glândulas exócrinas. Suas principais manifestações clínicas estão associadas à redução da função das glândulas salivares e lacrimais, que acarretam olhos e boca secos. Divide-se em forma glandular e extraglandular, sendo que cerca de um terço dos pacientes apresentam a manifestação extraglandular, caracterizada pelo acometimento sistêmico. Objetivo: Descrever as manifestações sistêmicas presentes na forma extraglandular da Síndrome de Sjogren. Material e métodos: Esta revisão de literatura foi realizada por meio de busca na base de dados eletrônica do PubMed com os seguintes termos “sjogren syndrome AND systemic”. Os filtros selecionados foram: textos completos gratuitos, humanos, publicados nos últimos 5 anos e disponíveis em português. Foram encontrados 387 resultados sendo selecionados 11 artigos para esta revisão de literatura. Resultados: A forma extraglandular da Síndrome de Sjogren está associada à fadiga fácil, febre baixa, mialgias, artralgias e episódios de artrite não erosiva. Outras manifestações incluem nefrite intersticial, linfadenopatias, hipostenúria, nefrocalcinose, vasculites, tromboangeíte obliterante, hepatomegalia, alterações pulmonares relacionadas a redução da secreção de glândulas mucosas no trato respiratório, Fenômeno de Raynaud, púrpura, urticária recorrente e neuropatias periféricas sensitivo-motoras. O linfoma representa a principal complicação associada à doença, sendo 15 a 20 vezes mais comum em relação à população normal. Conclusão: Verifica-se que a Síndrome de Sjogren acompanha um conjunto de manifestações glandulares e extraglandulares, sendo que dentro da forma extraglandular predominam as artralgias, Fenômeno de Raynaud, linfadenopatia e o envolvimento pulmonar. Reconhecer o acometimento sistêmico da doença é de suma importância, uma vez que possibilita uma melhor abordagem diagnóstica e evita possíveis complicações.
