IDENTIFICAÇÃO DA DOR NO PACIENTE GRAVE

Resumo
Introdução: A dor é uma experiência multidimensional complexa, envolvendo componentes sensoriais, fisiológicos e emocionais. Ao longo dos anos, foram elaboradas diversas escalas para avaliação da dor em pequenos animais, por meio de sinais comportamentais e físicos. Objetivo: Sintetizar as principais formas de identificação da dor em pacientes graves. Material e métodos: Trata-se de um estudo de natureza qualitativa utilizando levantamento de dados e revisão bibliográfica. Resultados: Um dos principais meios de avaliar a dor em animais é o uso de escalas, desde as mais simples e unidimensionais como a Escala de Classificação Numérica e a Escala Visual Analógica, que se baseiam em um formato de linha reta, numerada ou não, para dimensionar o grau de dor do paciente; e a Escala Descritiva Simples em formato de tabela, com as subclassificações de: ausência de dor, dor leve, dor moderada ou dor grave. Estas escalas exigem o registro de valores subjetivos para a intensidade da dor, que pode ser influenciado por fatores externos. Devido a essa variação, a fidedignidade dessas escalas é limitada. Com base nisso, escalas mais aprimoradas foram recentemente desenvolvidas, como a Escala de Dor da Universidade de Melbourne: baseada em respostas comportamentais e fisiológicas específicas, criteriosa para a avaliação pós-cirúrgica em cães. Outro método é a Escala de Dor da Universidade de Colorado: onde o paciente é avaliado em descanso, a uma distância que não o perturbe. Posteriormente, é avaliado o estado geral em resposta à palpação. Outra escala que trouxe grande impacto científico é a Escala Multidimensional da UNESP-Botucatu para avaliação de dor aguda em felinos, envolvendo alteração psicomotora, proteção da área dolorosa, variáveis fisiológicas e expressão vocal da dor. Ademais, a escala Short Form of the Glasgow Composite Pain Scale, que permite a avaliação de categorias comportamentais proporcionando uma melhor graduação da intensidade da dor do paciente, também é bastante utilizada. Evidencia-se que cada uma dessas escalas possui variação de pontuação própria, indicando o tratamento a partir de determinado estágio. Conclusão: As escalas de avaliação de dor facilitam a identificação da mesma, bem como o prognóstico do paciente e os possíveis tratamentos conforme sua intensidade.
