Introdução: O câncer de pulmão de não-pequenas células apical é uma das causas da Síndrome de Pancoast, a qual tem extensão local para o plexo braquial, cadeia simpática paravertebral, corpos vertebrais e primeira, segunda e terceira costelas. Expressa-se por dor no ombro e face dorsal do braço, ipsilateral ao tumor, dormência no cotovelo e antebraço, podendo evoluir com hipotrofia muscular ou até mesmo atrofia de 4º e 5º quirodáctilos. Objetivos: Relatar caso incomum de câncer de pulmão apical causador da Síndrome de Pancoast, investigação diagnóstica e abordagem terapêutica. Material e métodos: Este relato de caso se baseou em coleta de dados através da análise de prontuário no Hospital de Clínicas de Passo Fundo (RS), apresentação de resultados de exames complementares e revisão da literatura. Os dados do paciente serão mantidos em sigilo. Resultados: Paciente masculino, 74 anos, ex-tabagista, com história de câncer de reto ressecado há 7 anos, relatava dor de forte intensidade em ombro direito há 5 meses, acompanhada de astenia, perda ponderal de 6kg e anemia refratária ao tratamento. Ao exame físico apresentava murmúrios vesiculares diminuídos à direita. TC de tórax com achado de uma formação tissular de 3,8x2,5cm no ápice do pulmão direito, infiltração da pleura e linfonodo subcarinal de 3,6x1,6cm. Broncoscopia negativa para células neoplásicas e mediastinoscopia negativa para metástases linfonodais. Realizada lobectomia LSD, toracectomia de 1º, 2º e 3º arcos costais e linfadenectomia mediastinal. Através do estudo imuno-histoquímico da peça cirúrgica, foi estabelecido o diagnóstico de carcinoma de grandes células. Conclusão: O carcinoma pulmonar de não-pequenas células causador da síndrome de Pancoast é um tumor incomum e apresenta sinais e sintomas típicos de compressão do plexo braquial. Diante disso, a associação da clínica com exames de imagem é necessária para o diagnóstico e a conduta terapêutica é a ressecção do tumor.
