Introdução: No último século observou-se que a população dos países desenvolvidos e em desenvolvimento está se tornando fisicamente menos ativa, afetando negativamente a expectativa e qualidade de vida dessas populações, além da redução de exercícios físicos contribuírem para tal efeito, outros fatores são capazes de modificar o comportamento do sistema imunológico prejudicando a saúde do indivíduo, tais como: idade, fatores genéticos, metabólicos, ambientais, anatómicos, fisiológicos, nutricionais e microbiológicos. O destaque ao exercício físico justifica-se por tratar-se de um fator de risco modificável. Objetivo: descrever a influência dos exercícios físicos sob o sistema imunológico e suas implicações sob a saúde. Metodologia: O presente estudo trata-se de uma revisão bibliográfica da literatura realizada nas bases de dados indexadas a Biblioteca Virtual em Saúde (BVS) utilizando os descritores em português indexados ao DeCS: exercício físico, sistema imunológico e saúde associados ao operador booleano “and". Os critérios de inclusão foram textos completos disponíveis na íntegra nos idiomas inglês, português e espanhol publicados entre 2016 e 2021, que abordassem o tema em questão. Resultados: Na busca, utilizando os descritores, foi encontrado um total de 55 publicações, após a inserir os critérios de inclusão restaram 13 publicações que atendiam os critérios, das quais 8 estavam indexadas na MEDLINE e 5 na LILACS, após a leitura dos resumos e do texto na íntegra foram incluídos 5 artigos para a elaboração do presente estudo. Conclusão: É possível sugerir que exercícios físicos aeróbicos de intensidade moderada tem influenciado o sistema imune gerando neuroproteção contra altas concentrações de amônia no cérebro e também a modulação de marcadores inflamatórios e tem se mostrado uma importante ferramenta no manejo de alterações no sistema imunológico.
